I live in the U.S. and I have only seen that sleeve once. I don't know if it was only made in certain parts of the States. The red sleeve is far more common.

But... they did put it on the front cover of "Complete Motown Singles- 1965" from Hip-O so it was used at some point.

Definitely Rare, seen it once in a listing on ebay.

That was where I got it from, it was a month or so ago. It didnt sell and seller is local to me and I got it off him last week at Hamilton, Ontario show for $2 with Miracles - Shop Around on striped Tamla.
