You're right about 'Who Do You Think You Are'....I originally bought my copy for that side but 'Cold Hearted Blues' has become the side I play now. Anyway, interesting scan I've had 5 or 6 copies through my hands in the last couple of years and have seen about the same number in sales boxes etc but I've never seen one with the same label design as your scan. They have all been like this one:
