Yes, it was only on an LP, at least in USA and Canada.  I've never seen a "foreign" 45  or EP with it, but, I'm not an expert on non-US Motown releases.

Have in mind that the Brazil 45 is from 1974 or 1975.

Just dug out my six track seven inch EP of Down To Earth and damn it excludes the Angel Baby track. It does have the better (in my view) Hey Love track though. 

Really love the Brazilian one and it’s now on my wants....

It's the same as the one pictured above but white label. 'Not For Sale/Demo' markings on it.
