It's a Mac Rebennack (a.k.a Doctor John) track, under the name The Zu Zu Blues Band... covered as "Brown Sugar" by the Watts 103rd St Band.

Yep, seen a couple... Lack-lustre Jazz track that has crossed over from the R&B rooms to the Northern rooms. Can't understand why they've plumped for this one, when there are loads of crackin' uptempo R&B tracks that scream Northern at you...

I thought the extra holes were used for pinning the acetate in position on the 'lathe' - otherwise the disk would just spin uncontrolably when any amount of pressure was applied on the cutting tool
